What it is: two separate weekly grids on one page for a biweekly pay period, each with its own total, because overtime is counted week by week, never across the two weeks.

Why two totals and not one

45 hours in week 1 and 35 in week 2 is 80 hours, but it is still 5 hours of overtime, all in week 1. A single 14-day total hides that. Fill each week, total each week, and mark any hours over 40 as overtime before adding the weeks together.

What dates go in the pay period line?

The first and last day of the two-week period, for example Sunday the 5th to Saturday the 18th. The payday is usually a few days after the period ends.
