Filling in the regular and overtime columns
Put every hour in Regular until the week reaches 40, then the rest in Overtime. If you work 9 hours Monday to Thursday and 8 on Friday, Friday's first 4 hours are regular and the last 4 are overtime. Breaks of 5 to 20 minutes are paid and stay in the day; only a real meal break is written in the break column (29 CFR 785.18, 785.19).

What goes in the break column?
Unpaid meal breaks in minutes, usually 30 or 60. Short rest breaks are paid time and are not written as a deduction.
How do I split regular and overtime on a timesheet?
Count hours in order through the workweek. Everything up to 40 is regular; every hour after that is overtime, whichever day it falls on.
